1What is dentalwise?

dentalwise is the solution for all uninsured Australians who are committed to improving their dental health. We provide check-up options for patients who want to be able to budget for their dental check-ups as well as providing up front, transparent pricing on a range dental treatments..

Taking advantage of our easy monthly payments for a dentalwise plan means that you can plan for your preventative dental treatments while still being able to manage your budget.

2What is included in a check-up with dentalwise?

With dentalwise, you are choosing the manage the cost of either one check-up or two check-ups a year with your dentalwise plan. Included in your check-up are the following items: dental examination (011, 012), scale and clean (111) or removal of plaque (114), fluoride application (121), X-ray (2 x 022) and OPG (037).

All services are provided by independent practitioners who participate in dentalwise. Services will only be provided where clinically necessary in the clinical opinion of the dentist.

3How does a dentalwise plan work?

dentalwise plans allow you to budget for the cost of your regular check-ups, by paying the cost of your check-ups to dentalwise over each year.

When you hold a dentalwise plan:

• you are entitled to the capped prices for regular check-ups with participating dentists at your nominated centre. Capped prices quoted are as specified in your selected dentalwise plan;

• you make regular payments to dentalwise equal to the capped price of either one or two regular check-ups (as specified in your selected dentalwise plan) over the course of each 12 month period commencing on the date you acquire your plan;

• when you attend a participating dentist at your nominated centre and receive your regular check-up, dentalwise will pay your dentist on your behalf (even if you have not yet paid to dentalwise the full amount of that check-up), up to the number of regular check-ups specified in your dentalwise plan, with a maximum of one regular check-up payable by dentalwise on behalf of each person in any 6 month period.

In addition, persons who hold a dentalwise plan are entitled to discounted rates off the capped prices for dental treatments at your nominated centre (such capped prices are the capped prices available to persons who do not have private health insurance).

4How do I manage the cost of my dental check ups with dentalwise?

Payments are made to dentalwise by direct debit. You can make 'up-front' payments, for the entire cost of your regular check-ups at the start of each year, or you can make on initial $50 payment on the day you commence with dentalwise and then pay via monthly deposits equal to the cost of your regular check-ups over the course of the year.

5Who provides the dental services to me?

All participating dentists are independent practitioners at selected Pacific Smiles Dental centres and all dental services are provided in the absolute discretion of that independent practitioner. A dentalwise plan does not guarantee you will receive dental services from your preferred participating dentist or that you are eligible to receive a regular check-up or any other dental service.

6What are the prices for any other dental services I may need in addition to my dental check ups?

Members are also entitled to the specified discount on capped pricing on dental treatments for persons without private health insurance with participating dentists at their nominated centre.

Members who choose a Sensible plan will receive a 5% discount on additional dental treatments, Members who choose a Smart plan will receive a 10% discount. (excluding orthodontics)

7Is there a waiting period?

No, you can attend a participating dentist at your nominated centre at any time once you have signed on for a dentalwise plan (subject to clinical need).

The capped prices quoted are available for regular check-ups with participating dentists at a person's nominated centre, to individuals who enter into an agreement with dentalwise to pay for their regular check-ups through a dentalwise plan (Members). Capped prices available will depend on the dentalwise plan which a Member selects (Sensible or Smart). Members will select to pay dentalwise for either one or two regular check-ups with participating dentists at a Member's nominated centre, per 12 month period calculated from the commencement date of the dentalwise plan. When a Member attend a participating dentist at their nominated centre and receive their regular check-up, dentalwise will pay the Member's dentist on the Member's behalf (even if they have not yet paid to dentalwise the full amount of that check-up), up to the number of regular check-ups specified in the Member's dentalwise plan, with a maximum of one regular check-up payable by dentalwise on behalf of each person in any 6 month period.

Members are also entitled to the specified discount on capped pricing on a range of dental treatments for persons without private health insurance with participating dentists at their nominated centre. Where three of more people enter into an agreement with dentalwise for a dentalwise plan at the same time, those Members are entitled to a further discount on capped prices for regular check-ups.

Membership, and any benefits which flow from therefrom (including capped prices) are not available to persons who hold private health insurance coverage with respect to dental services. Capped pricing cannot be used in conjunction with any other offer. There is a limit of one dentalwise plan per person. Dental services, including any regular check-ups, are provided at the absolute discretion of the participating dentists, and will only be provided where clinically necessary and appropriate for eligible patients.

Unless we or a Member cancel a dentalwise plan, dentalwise plans will automatically renew each year. dentalwise will continue to deduct payments from a Member's nominated account in accordance with their dentalwise plan until the plan is cancelled. However, while a dentalwise plan does not expire, subject to our discretion, dentalwise does not generally refund any amounts paid to it for dental services which are not used in a relevant year.

A Member can cancel their dentalwise plan at any time. Generally, if a Member cancels a dentalwise plan (other than due to a variation of the dentalwise plan by dentalwise or due to clinical need) when they have not attended their participating dentist for one or more regular check-ups for the current plan year, (such that the Member has paid more to dentalwise for the current plan year than dentalwise has paid to a participating dentist on the Member's behalf for the current plan year), the Member may either:

Members will receive a refund of any amounts they have paid to dentalwise in the current plan year in excess of the administration fee.

If a Member cancels a dentalwise plan due to a variation of the dentalwise plan by dentalwise or due to clinical need, the Member will only be required to pay to dentalwise any amount dentalwise has paid to a participating dentist on their behalf for the current plan year.

If, at the time a Member cancels, dentalwise has paid their participating dentist for one or more regular check-ups in the current plan year, and the Member has not paid dentalwise the full cost of those check-ups, the Member must immediately pay any outstanding amount to dentalwise. For the avoidance of doubt, no administration fee is payable in this circumstance.

Any amount owing to dentalwise, including any amount due on cancellation will be deducted from amounts paid to dentalwise in a plan year, or direct debited from a Member's nominated account.

In these terms 'regular check-up' means a service comprising 1 standard dental check-up, professional teeth cleaning and fluoride, and, where applicable x-rays.
